Annual Student Information Form – Action Required

Please be on the lookout for our annual returning student contact information form. This form will be sent via email and look like the image you see above.

This annual form allows families to review, confirm, and update important student and household information in our Student Information System.

The form also includes required acknowledgments and sign-offs for important district information, including student handbooks, the Technology Responsible Use Policy, medical information, and other school policies and permissions.

Please note: A parent/guardian must acknowledge the Technology Responsible Use Policy for their student to access district technology resources. Students without a completed acknowledgment will lose access to their school-issued account, device, and the school network.

Please complete the form as soon as possible to ensure your student's information is accurate and that they maintain access to all necessary school resources.
