The Middle School offers sports opportunities for students in 7th and 8th grade. The program has been established to provide a fun learning experience for all students. Students will need insurance coverage through a home policy or the school insurance program to participate.

Middle School sports include: boys and girls soccer, girls volleyball, boys and girls cross country, coed golf, girls softball, and baseball. 7th and 8th grade students may also participate in freshman football and basketball. During the 2016-17 school year there will also be a Middle School Football team for the first time.

In order for a middle school student to participate in school sports, s/he must be sure to register at https://www.familyid.com/millis-athletics 

PHYSICAL EXAM: ***PHYSICAL EXAMS EXPIRE 13 MONTHS FROM THE DATE THE EXAM WAS ADMINISTERED. A copy of the athlete's current physical exam must be on file with the Athletics Department. Please submit a copy if you have not done so already. 

**Should the physical exam expire during the season, the athlete will be ineligible to participate until a new current physical exam form is submitted to the Athletic Department. NO EXCEPTIONS will be made. PLEASE be aware of when it expires and make appointments well in advance. 

PLEASE NOTE: If the physical exam date entered in the online registration does not match the date on the actual physical exam form provided to the school, eligibility may be affected! 


ANNUAL CONCUSSION EDUCATION: 

EACH athlete/band member AND a parent or guardian MUST complete the online concussion course AND submit a completion certificate to the Athletics Department ANNUALLY, per state law. 

**A COMPLETION CERTIFICATE FOR ONE OF THE ONLINE COURSES MUST BE SUBMITTED IF IT HAS BEEN A YEAR OR MORE SINCE LAST SUBMITTED (i.e. Fall 2016 or earlier). 

If you are not sure when one was last submitted, please submit a new one. 

ONLINE COURSE options: ***EITHER COURSE IS ACCEPTABLE! Both are free. (Please enter ALL names of those watching the video when you register for/order it!)

5th and 6th graders who wish to participate in sports should contact the Millis Recreation Department for information.
